All-on-6 or All-on-4: how to choose?

It’s one of the questions that come up most often. The choice depends on your anatomy and your needs.

All-on-6, with its two extra implants, spreads the biting forces better and increases the contact surface between bone and implants. That’s a real asset for the upper arch (the maxilla), where the bone is naturally less dense. Patients who suffer from bruxism (teeth grinding) also benefit from this more even distribution of strain.

The large study by Caramês and colleagues (2025), which followed 943 patients and 5989 implants over an average of 5 years, reports almost identical 5-year survival rates between the two setups: 98.8% for All-on-4 and 98.7% for All-on-6. The edge tips slightly towards six implants in the upper jaw, precisely where the bone poses the most difficulty. In other words, both techniques are excellent; All-on-6 mainly adds a biomechanical safety margin where the anatomy calls for it.

A recent multicentre clinical trial (Toia and colleagues, 2025) reports a 5-year implant survival rate of 99.3% in the six-implant group.
