When a breast reduction is the right call

Reduction mammoplasty answers several situations. The main one is breast hypertrophy: breasts that are too large for your build and genuinely weigh on daily life. Here are the reasons that come up most often in consultation:

The breast reduction techniques we use

The inverted-T technique (anchor scar)

This is the most common technique, made for larger reductions (beyond 400 to 500g per breast). The surgeon makes three incisions: around the areola, vertically down to the fold, then horizontally along the crease under the breast. It removes a good amount of tissue while reshaping a balanced chest.

The scars are more extensive, but placed where they show least: the change in colour around the areola masks one, the fold under the breast and your bra handle the rest. With the right care, they fade noticeably over the months.

The vertical technique (I-shaped scar)

For mild to moderate hypertrophy, this approach keeps scarring to two areas: around the areola and a vertical line down to the fold. No horizontal scar, then, which is a real cosmetic plus. The breast you end up with is nicely shaped, with a natural projection.

Worth knowing: healing is often quicker than with the inverted-T. Your surgeon checks in consultation that your anatomy and the amount to remove are a good fit.

The peri-areolar technique (round block)

Kept for the lightest reductions, it leaves just one discreet scar around the areola, right where the darker and lighter skin meet. It suits a modest excess paired with slight sagging. The amount removed stays limited, but the scar result is especially discreet.

The superior pedicle technique

This method protects the blood supply and nerves of the nipple through a bridge of tissue above it. Nipple sensation holds up well in the long run. If you hope to breastfeed one day, your surgeon may prefer an inferior pedicle, which keeps the milk ducts safer.

What happens during surgery at our partner clinic

A breast reduction takes place under general anaesthetic, in one of the theatres at our Istanbul clinic. Allow 1.5 to 3 hours depending on how much needs removing and the technique chosen. The operation runs in four stages.

Pre-op marking. Done standing on the morning of surgery, it maps out precisely the areas to remove, the new position of the areola and the incision lines. This is what sets the symmetry of the result.

Removal. The surgeon takes away the excess gland, fat and skin according to the plan. The amount varies a lot from one patient to the next, from a few hundred grams to more than 1000g per breast for severe hypertrophy.

Reshaping. The breast is sculpted back to a natural shape. The areola is repositioned at the right height and, if needed, reduced in diameter to stay in proportion.

Closing up. The stitches go in across several layers, usually with dissolvable sutures. A compression dressing and a surgical support bra go on straight away.

One night in hospital is planned, to check the early hours pass smoothly. Drains may be placed, then taken out the next day.

Aftercare and recovery

Pain after surgery is usually moderate and well controlled with painkillers. Some swelling and bruising is normal in the first two weeks: it settles on its own.

The surgical support bra is worn day and night for 4 to 6 weeks. It holds the new shape and helps clean healing, so it’s worth sticking to. As the sutures are usually dissolvable, there’s generally no stitch removal to arrange.

From the third or fourth week, once the scars have closed, scar massage comes in. Done daily and kept up for at least a month, it softens the tissue and improves the final look. Here, broadly, is the pace at which to pick things back up:

• Light daily activities: 7 to 10 days
• Desk work: 1 to 2 weeks
• Driving: 2 to 3 weeks
• Physical work: 4 to 6 weeks
• Intense sport: 6 to 8 weeks
• Final result: 3 to 6 months
