How does the FUE technique work?

The principle: extracting one follicle at a time

FUE (follicular unit extraction) rests on a simple idea, though one that demands real precision. The surgeon removes each follicular unit one by one from the donor area (the back and sides of the head), using a micro-punch between 0.7 and 1mm across. Each graft, made up of 1 to 4 hairs, is then kept in a temperature-controlled solution before being implanted into the thinning area.

That’s the big difference from the older FUT method, which meant removing a whole strip of scalp and left a visible linear scar. With FUE, the only marks are tiny dots under a millimetre wide, hidden as soon as the hair grows back.

Recent clinical studies back up how well this works. A study of 158 patients found a follicle survival rate above 90% at 12 months, with more than 85% of patients passing the 95% survival mark (Wang et al., 2024).

FUE, Sapphire FUE, DHI: three variants, one goal

Are you a good candidate for FUE?

The patients we see in Istanbul

Most of our patients are men affected by male pattern baldness, the gradual, hereditary hair loss known as androgenetic alopecia. It’s graded on the Norwood scale, from stage II (a receding hairline at the temples) to stage VII (complete baldness on top). FUE is suitable from stage II, once the hair loss has settled.

Women make up a growing share of our patients in Istanbul. Female hair loss shows up as diffuse thinning, graded on the Ludwig scale (stages I to III). Who is eligible

You need to be at least 25 and your hair loss should have stabilised. The quality of your donor area is decisive, and your surgeon assesses it during the first consultation. Some conditions rule the procedure out: uncontrolled clotting disorders, autoimmune conditions of the scalp, active scalp infections. Poorly controlled diabetes and heavy smoking are relative contraindications.

Your FUE hair transplant in Istanbul, step by step

Design and preparation

It all starts with drawing your new hairline, right there at the clinic in Istanbul. This is a key moment: that outline decides how natural the final result looks. Your surgeon takes account of the shape of your face, your age and how your hair loss is likely to progress. The donor area is then shaved and local anaesthetic is given. Removing the grafts

The surgeon removes the follicles one by one with precision micro-punches. Each graft is sorted under a microscope to pick out the strongest follicular units. If your surgeon recommends it, the grafts can also be treated with PRP (platelet-rich plasma): a small amount of your blood is spun in a centrifuge to isolate the plasma, which is then applied to the grafts to help them take and grow.

Opening the channels and implanting

The surgeon opens the implantation channels in the recipient area (except with DHI, where this step and the implantation are combined). Each incision is exactly as deep as the follicle is long, and the angle is worked out to copy the natural direction of growth. Every graft goes in by hand, one at a time. Where FUE really wins

The first benefit is that there’s no linear scar. Where FUT left a visible mark under short haircuts, FUE leaves only tiny dots under a millimetre wide, undetectable once the hair grows back. Recovery is quick: most patients are back to their normal routine after 2 to 3 days. Redness fades within a week, and the scabs drop off in 10 to 14 days.

FUE also lets the surgeon widen the harvest to the temples, the beard, or even parts of the body if the scalp alone isn’t enough. And because the grafts go in one by one, the density, angle and direction of every hair can be controlled precisely for a natural look.

What to bear in mind

FUE usually means shaving the whole head. The density it achieves is slightly lower than with DHI, which is why it sometimes makes sense to combine the two. Complications stay rare: the overall rate is put at between 1.2 and 4.7%, and the large majority are minor (Romera de Blas et al., 2026). Regrowth: what to expect month by month

About 80% of the transplanted hairs fall out between weeks 3 and 6. This is the well-known shock loss, and it’s completely normal. From their intact roots, new hairs, fine as down at first, start to appear from the third month. Between the sixth and ninth months, the hair thickens up. The final result shows between 12 and 18 months. The transplanted follicles come from the back of the scalp, which is genetically resistant to balding, so they keep growing for life.
