Understanding breast sagging and how to treat it

Breasts that drop after a pregnancy, volume that fades once you stop breastfeeding, skin that loosens after a diet or simply with age: breast ptosis, the medical name for sagging breasts, affects a lot of women, at every age.     Kept for mild sagging, this technique involves a single incision around the areola. Hidden at the edge of the areola, the scar soon fades to something very discreet.

  • Vertical scar

    For moderate ptosis, the incision goes around the areola and then straight down to the crease under the breast. It gives a lovely lift while staying hidden under a bra.

  • Inverted-T scar

    Used for significant sagging, this adds a horizontal incision along the crease under the breast. It’s the technique that removes the most excess skin, with a scar that fades noticeably over the months.

What’s the difference between a breast lift and a breast augmentation?

A breast lift (mastopexy) raises and reshapes sagging breasts without changing their volume. A breast augmentation sets out to increase the volume of the breasts, usually with implants or fat transfer. The two operations can be combined when a woman has both sagging and a loss of volume.

Can you breastfeed after a breast lift?

Breastfeeding often remains possible, especially with a superomedial pedicle that keeps the nipple attached to the gland, but it is not certain. Newer techniques aim to preserve the milk ducts and nipple sensation. Tell your surgeon if you still plan to breastfeed, so they can adapt the approach.

How long is the recovery?

Pain afterwards is usually mild to moderate and well controlled with ordinary painkillers. Some swelling lingers for about 2 weeks. Most women return to work after 7 to 10 days, and to sport after 6 weeks at the earliest. You’ll need to wear a support bra for 4 to 6 weeks.

Can a breast lift correct asymmetry?

Yes. A breast lift works particularly well for a pronounced breast asymmetry. The surgeon adapts the technique used on each breast to reach a symmetrical, balanced result. A careful assessment at the consultation sets out the surgical plan that fits you.

Are the scars visible?

It depends on the technique. The periareolar scar on its own is very discreet. The vertical scar stays well hidden under a bra. The inverted-T scar is more extensive but fades considerably with time and the right care. Our surgeons use advanced suturing techniques to get the best possible scars.

What are the possible risks and complications?

Like any surgery, a breast lift carries some risks: a haematoma, infection, delayed healing, more visible scars, a temporary change in nipple sensation, or the risks tied to anaesthetic. What age can you have a breast lift?

A breast lift can be done once the breasts have finished developing, around 18. It’s usually better to wait until you’ve finished having children, though, since pregnancy and breastfeeding can change the result. There’s no upper age limit, as long as your health allows it.

Une chirurgie qui a beaucoup progressé

Le lifting mammaire n’a plus grand-chose à voir avec ce qu’il était il y a quarante ans. Les techniques modernes de remodelage, une meilleure compréhension de la vascularisation du sein et des sutures plus fines ont changé la donne : cicatrices plus courtes, suites plus simples, résultats plus naturels. La classification de Regnault : mesurer l’affaissement

Pour choisir la bonne technique, le chirurgien évalue d’abord le degré de ptose. La référence reste la classification de Regnault, qui compare la position du mamelon à celle du sillon sous-mammaire, le pli naturel situé sous le sein.

  • Grade 1, ptose légère : le mamelon arrive au niveau du sillon, le bas du sein commence tout juste à s’affaisser.
  • Grade 2, ptose modérée : le mamelon passe sous le sillon, mais reste au-dessus du point le plus bas du sein.
  • Grade 3, ptose sévère : le mamelon pointe vers le bas, au point le plus bas du sein, avec un excès de peau marqué.

Il existe aussi la pseudoptose, fréquente après l’allaitement : le mamelon reste bien placé, mais le sein se vide par le bas et paraît dégonflé. Ce cas se corrige souvent par une augmentation mammaire seule, sans lifting. C’est cette évaluation qui oriente vers une cicatrice périaréolaire, verticale ou en T inversé.

Des techniques de moins en moins invasives

La chirurgie mammaire continue d’avancer. Des équipes ont récemment décrit une mastopexie interne, qui rehausse le sein en travaillant entre la glande et le muscle, sans la cicatrice en T inversé. L’approche séduit surtout les patientes jeunes, avec une ptose légère et sans projet de grossesse en cours.

Pour les affaissements débutants, des méthodes mini-invasives se développent aussi : fils résorbables, radiofréquence, petites incisions. Elles ne remplacent pas la chirurgie sur une ptose marquée, mais elles élargissent le choix pour celles qui veulent une correction discrète et une récupération rapide. Ce que disent les résultats

Sur une série de 500 augmentations-mastopexies suivies par voie verticale, les complications tournaient autour de 6%, et aucune n’était considérée comme grave.

Les complications les plus souvent rapportées concernent la cicatrice et le complexe aréolo-mamelonnaire, plus que l’hématome ou l’infection. Rien d’alarmant, donc, mais rien à négliger non plus : le respect des consignes avant et après l’opération fait une vraie différence.

La durée des résultats, elle, dépend de vous : un poids stable, l’absence de nouvelle grossesse, une peau bien hydratée et des cicatrices soignées. C’est d’ailleurs pourquoi on conseille souvent d’attendre la fin de vos projets de grossesse avant de vous lancer.

Mastopexie et interventions combinées

Le lifting mammaire se marie bien avec d’autres gestes. Quand le sein manque aussi de volume, on l’associe à une augmentation mammaire par implants ou à un lipofilling dans le même temps opératoire.
