Back lift: understanding the procedure

The back is one of the few areas you never really see head-on, yet it’s often the first to give away significant weight loss or the passing years. The rolls that spill over the bra strap, the horizontal folds and the skin that has lost its firmness tend to hold out against exercise and dieting, because they come down to loose skin that no amount of training can tighten. The back lift, known in the surgical literature as a bra-line back lift, was designed for exactly this.

Why the skin on the back slackens

The skin on the back is unusually thick and made up of three layers: the epidermis on the surface, the dermis that gives it its elasticity, and the hypodermis, which holds the fatty tissue. With age, collagen and elastin fibres thin out and the skin slowly loses its ability to spring back. After major weight loss the effect is stronger still: stretched for so long, the skin no longer retracts over what is now a smaller frame.

Fat on the back settles in a few well-defined spots our surgeons know well. It gathers mainly below the shoulder blades, just above the waist, across the upper back near the base of the neck, and under the arms, where the side roll of the bra strap continues. These match up with natural zones of adhesion, where the skin is anchored more firmly to the deeper layers, and that’s exactly what creates the folds and stops lower-body lifts from doing anything for the upper back.

The classic lift comes into its own when there’s plenty of loose skin and only moderate fat: the horizontal incision then follows the bra line to tuck the scar out of sight. Liposuction on its own, by contrast, is enough when the fat is pronounced but the skin still has good tone, the deposits drawn out through fine cannulas via tiny incisions. And after major weight loss, the extended lift carries the incision round towards the flanks to redraw the whole upper trunk. When the skin still has enough tone to skip a wide excision, radiofrequency-assisted liposuction can help it retract more than standard liposuction (Theodorou et al., 2018).

How the operation is done

Carried out under general anaesthetic, the back lift takes 2 to 3 hours on average. It usually starts with liposuction of the fatty deposits, followed by careful removal of the excess skin along markings drawn while you’re standing, the day before or the morning of surgery. The surgeon then closes the wound with deep, layered sutures, a technique that eases the tension on the scar and lowers the risk of fluid build-up (seroma). The final scar tucks discreetly into the crease of the bra line, which makes it easy to hide.

Recovery and results

Your hospital stay runs to one or two nights, followed by a compression garment worn for several weeks to support the tissues and bring down swelling. Things pick up in stages: gentle activity is fine again after a couple of weeks, work usually falls between the second and third week depending on your job, and sport can restart gradually from the sixth week.
