Calf liposuction
Calf liposuction reshapes your lower legs for good by removing the stubborn fat that won’t shift with diet or sport. This targeted procedure redraws the line of your calves for a slimmer, more balanced silhouette.
Calf liposuction at a glance
Your procedure, step by step
Your surgeon works out what your calves are made of using the skin pinch test and, if needed, an ultrasound scan. This step matters: it confirms that the volume of your calves really comes from excess fat, and not from developed muscle or long-standing swelling. You leave with a clear treatment plan and a detailed quote.
You’ll have a full set of medical tests to rule out any reason not to go ahead. Your surgeon talks you through what to do beforehand: stopping smoking 4 weeks before the operation, coming off certain blood-thinning medicines, and arranging your stay in Istanbul with your dedicated Body Expert assistant.
Done under tumescent local anaesthetic or general anaesthetic depending on the case, the operation takes 1.5 to 2 hours. Through tiny 3 to 5 mm incisions tucked into the natural folds of the skin, the surgeon slips in fine, round-tipped cannulas that draw off the fat evenly, all the way around the calf.
Compression stockings (or sleeves) go on as soon as the operation finishes and need to be worn day and night for 4 to 6 weeks. Depending on your risk profile, blood-thinning injections may be prescribed to guard against the risk of a clot. You can have your first shower the very next day.
You’ll ease back into everyday activities over 5 to 15 days. Hard effort and sport are best left for about 2 months. The post-operative swelling, at its worst around day 7, settles gradually. The natural result shows through at around 3 months, and the final result is there to judge between 6 and 12 months.
Have your calves bothered you for years? Nothing you try, neither sport nor dieting, seems to slim them down? That situation is very common. When the volume of your calves comes from a constitutional layer of fat, only a targeted operation can deal with it.
Calf liposuction (or calf lipoaspiration) is a minimally invasive cosmetic procedure that permanently draws off the surplus fat cells sitting around the calves, and sometimes the ankles. Carried out through tiny incisions of 3 to 5 mm, it redraws the natural curve of your lower legs with very discreet scars. The skin over the calf, naturally thick and elastic, tightens gradually after the fat is removed and settles onto your new contours.
This operation is only for calves whose volume comes from excess fat under the skin. If your calves are large because of well-developed muscle or long-standing swelling (heavy legs, poor venous return), liposuction isn’t the answer. Only a qualified plastic surgeon can make that call, at a dedicated consultation, largely thanks to the skin pinch test.
At Body Expert, our surgeons specialise in body contouring and handle the full range of liposuction techniques suited to the tricky area of the lower leg: tumescent liposuction, VASER, laser-assisted or microcannula liposuction. The technique is matched to your build, the amount of fat to remove and the quality of your skin, for a result made to measure.
ISAPS figures put liposuction among the most performed cosmetic procedures in the world, with around 2.1 million carried out in 2024. Liposuction in Turkey is very much part of that trend. The calves and ankles remain a delicate area: the fat layer is thin, and a good result depends on the indication being right.

Our prices and packages
Feel free to scroll through the different prices from right to left!
In the UK, calf liposuction is a sizeable outlay, and the bill climbs quickly once you add on the extras: the compression garment, follow-up consultations, nights in hospital. Because it’s a purely cosmetic operation, the NHS won’t cover any of it.
That’s exactly what leads many patients to look towards Istanbul. At Body Expert, you get up to 70% savings compared with UK prices, in a genuinely all-inclusive package. The figures, package by package, are set out in our price list higher up this page.
That package pulls into a single price what’s often billed separately elsewhere: the operation and the anaesthetic, the surgeon’s fees, the pre and post-operative care, the compression garment, transfers between the airport, the clinic and the hotel, and your accommodation while you’re there.
The sums add up fast. And if you make the most of the trip to treat several areas at once, such as the knees, the thighs or the ankles, the overall price becomes better still, while giving a balanced result across the whole leg.

Common questions about calf liposuction
Is calf liposuction painful?
The discomfort after the operation is usually moderate and well controlled with ordinary painkillers. The procedure itself is done under anaesthetic, so you feel nothing during the operation. Any post-operative soreness eases off over a few days.
How do I know if my large calves are down to fat or muscle?
Only a plastic surgeon can tell you for certain. At the consultation, they carry out the skin pinch test to gauge the thickness of the fat layer, and can order an ultrasound scan if needed. If the volume is muscular, calf liposuction simply isn’t indicated.
When can I start exercising again after the operation?
Expect to leave sport for about 2 months. Everyday movements can resume gradually after 5 to 15 days, depending on how much liposuction you’ve had. Your surgeon will tailor this advice to your situation.
Are the results really permanent?
The fat cells that are drawn off are gone for good and don’t grow back. So the results last, as long as you keep your weight steady. If you put on a lot of weight, the remaining cells could store more fat, but far more slowly than before.
Is there a risk of visible scars?
The incisions are 3 to 5 mm and sit in the natural folds of the skin. They fade to become very discreet and are usually hard to spot after a few months. The stitches dissolve on their own, so there’s nothing to take out.
Can calf liposuction be combined with other areas?
How long does the post-operative swelling last?
The swelling peaks around day 7 then eases off gradually. It can linger, mildly, for about 3 months. Manual lymphatic drainage speeds up its disappearance. The final result is there to judge between 6 and 12 months.

Let’s discuss your project
For a lot of people, the calves are a lasting source of self-consciousness. You can run, swim, watch what you eat, and still nothing changes: some calves flatly refuse to slim down. There’s a reason for that. When the volume comes from fat rather than muscle, neither sport nor dieting can target the area effectively. This is where calf liposuction comes in, permanently clearing the fat your body insists on storing in your lower legs.
Large calves come down to three main causes. The first, and the only one that liposuction addresses, is excess fat under the skin. It’s common in women, sometimes even in very slim people, because this shape is often constitutional and runs in families. The second cause is very well-developed muscle, with long muscles that run low towards the ankles. In that case, no amount of liposuction will slim the legs. The third cause is long-standing swelling of venous or lymphatic origin, which first needs medical treatment (venotonics, lymphatic drainage).
Only a clinical examination by a qualified plastic surgeon can tell these three situations apart. The surgeon carries out the skin pinch test, usually with you standing and then sitting so the calf muscle can relax. They take hold of the skin between thumb and forefinger to gauge the true thickness of the fat layer. This step, often backed up by an ultrasound scan, is what the success of the operation hinges on.
When the indication is right, meaning the volume genuinely comes from fat, the results can be very satisfying. This area remains delicate, though: the fat layer is thin, with dense fibrous tissue, and the risk of contour irregularities is higher than elsewhere, which calls for a careful technique.
Beyond the visible slimming, many patients describe a real change in daily life. Being able to pull on boots, or wear a skirt or a dress without a second thought, is a lift in confidence that goes well beyond looks. A prospective study in the Journal of Plastic, Reconstructive & Aesthetic Surgery found that cosmetic liposuction significantly improves body satisfaction and emotional stability, and lowers patients’ anxiety (Papadopulos et al., 2019).
The ideal candidates are adults in good general health, close to their natural weight, with fat sitting specifically on the calves and skin that has kept its elasticity. It’s best to wait until adulthood, and ideally until growth has fully finished. The operation gives its best results before 45 to 50, while the skin still has plenty of spring. If you’re planning a pregnancy or a big weight loss, it’s better to put your project off.
On the other hand, this operation isn’t right for people whose calf volume is down to muscle or to circulation problems. It doesn’t treat cellulite or loose skin either.
The results are there straight away at the end of the operation, even if the post-operative swelling hides the final look for a while. The natural shape of your slimmer calves starts to come through after about 3 months, as the swelling settles and the skin tightens. The final result is there to judge between 6 and 12 months after surgery.
The good news is that the fat cells drawn off don’t come back. So the results are permanent, provided of course that you keep your weight steady and your lifestyle balanced. If you gain a significant amount of weight, the remaining cells could store more fat, but far more slowly and diffusely than before the operation. Follow-up consultations are booked at 1, 3, 6 and 12 months to track your progress.
Like any surgery, calf liposuction carries risks. Infection after liposuction remains uncommon, with reported rates below 1% (Dixit & Wagh, 2013). According to ISAPS figures, liposuction is among the most performed procedures in cosmetic surgery, with around 2.1 million carried out worldwide in 2024.
The most common side effects are bruising, swelling of the treated areas that can last around 3 months, and moderate pain that painkillers control easily. Less often, you might see haematomas, infections (routinely headed off with antibiotics) or surface irregularities. The risk of a clot, particular to this area, is why blood thinners are prescribed as a matter of course for 10 to 15 days after the operation. And if you smoke, stopping at least 4 weeks before and after is strongly advised, as tobacco sharply raises the risk of complications and slows healing.

Calf liposuction, or lipoaspiration of the lower legs, is for people whose calves stay large because of localised excess fat that holds out against sport and dieting alike. It isn’t for muscular calves or for legs swollen by a venous problem: hence the importance of a precise diagnosis, made by a plastic surgeon at the consultation. When the indication is right, the results last, provided the diagnosis is precise and the technique stays conservative.
In practical terms, the operation takes 1.5 to 2 hours, is done as a day case through fine 3 to 5 mm incisions, and comes with a compression garment worn for a few weeks. The first slimmer contours show at the end of the operation, the natural result settles in around month 3, and the final look is there to judge between 6 and 12 months. The fat cells removed don’t return: at a steady weight, the benefit is yours for good.
